Very effective and caring MD. Has never steered us wrong. My family and I have been with him for over 15 years and feel grateful for his guidance and expertise.

A nice but fundamentally ineffective doctor. Horrible for punctuality - I have never waited less than an hour over my original appointment time. His medical knowledge is lagging, and there are fundamental gaps in his ability.

Doctor Kozak diagnosed me with ADD over a year ago and although I have having a very serious side effect as a result of my current medication he has me on. He took it lightly and advised me to take an additional drug to counteract it rather than switching me altogether

The receptionists are terrible and do not return phone calls. They act like dealing with clients are a pain.
Doctor Kozak is very nice and helpful but he seems to be off work more often than he's on.

I will lose a loved one because of his negligence. I will spare the details. Do yourself and your family a favour and go to a different doctor. Always seek a 2nd opinion especially if the doctor has you leaving with unanswered questions or any uneasiness. Seeing these other ratings for the first time shows me I am not alone in this thinking.

Dr. Kozak ignored symptoms of kiddney failure and passed them on as the flu. My immediate and extended family stopped going to him after being with him for 20 years. Found another fantastic Dr. and realized what I had been missing all these years. When my appointment states a time, I am seen within 10 minutes of that.
