It is impossible to get any information from this clinic once you leave.... They will put you on hold forever... and once they answer the phone, they will put you on hold again. Be sure you get everything you need to file an insurance claim BEFORE you leave their office or you will get NOTHING and NOWHERE with them.... the staff is not at all helpful!

This clinic is a mill.  The owner hires incompetent reception staff that make constant errors and he doesn't care about feedback.  He is obviously in it only for the money.  You have to be alert and your own advocate if you care about your health.One star is too many.
